Functional Levels
In addition to the Personal History Form, you also need to look at your loved one’s functional level. When planning meaningful activities based on individual interests, you need to also consider your loved one’s functional abilities. You need to set them up for success based on what they are able to accomplish. There are several definitions of functional levels. For the purposes of this topic, we will address the following four functioning levels:
Level 1
Your loved one has good social skills. They are able to communicate. They are alert and oriented to person, place and time, and they have a long attention span.
Level 2
Your loved one has less social skills and their verbal skills may be impaired as well. Your loved one may have some behavior symptoms. They may need something to do, and may have an increased energy level, but they have a shorter attention span.
Level 3
Your loved one has less social skills. Their verbal skills are even more impaired than they were at Level 2. They are also easily distracted. Your loved one may have some visual/spatial perception and balance concerns, and they need maximum assistance with their care.
Level 4
Your loved one has a low energy level, nonverbal communication skills, and they rarely initiate contact with others, however, they may respond if given time and cues.
With the personal history and functional level information, you and every caregiver have the greatest opportunity to provide person- appropriate activities for your loved one.
